About Your First Visit

At Era Health Doctor Melbourne Central Business District, we are committed to helping you to improve your health.

We provide consultations by appointment but will see walk-in patients if we have available appointments. Please note that we are a mixed (mostly private) billing clinic and that settlement of accounts is required at the time of your consultation.

What to expect on your first visit to our Melbourne Central Business District Doctors Clinic?

When you arrive at our door for your first visit, you’ll be welcomed by one of our friendly receptionists who will assist you throughout and help you in the entire process.

You will need to complete a new patient information form with some basic information about you and your health history.

We have three different types of forms depending on the reason for your visit.

If you are coming to see one of our Melbourne Central Business District general practitioners, physiotherapists, podiatrists, or psychologists, you will need to fill out a form with your health history.

If the reason for your visit is to see our Melbourne Central Business District dentist, a special dental form will need to be completed.
When you need to have a Melbourne Central Business District pre-employment medical or a work injury in Melbourne, an occupational health form will need to be completed.

You will be requested to arrive 10 minutes prior to your appointment to complete any of the new patient forms at the reception.
