MyMedicare

Era Health Doctor Melbourne CBD is now registered in MyMedicare.

Did you know that maintaining regular visits with the same GP can contribute to improved health outcomes? Introducing MyMedicare, a new patient registration model designed to enhance the delivery of medical care as your preferred primary health care team.

By enrolling in MyMedicare, you unlock benefits like extended funded GP telehealth consultations with our practice. Participation in MyMedicare is entirely voluntary and free of charge.

This platform empowers you to register with your chosen general practice, preferred GP, and primary care team, promoting greater continuity of care. Importantly, MyMedicare doesn’t restrict you to a specific healthcare provider, allowing you to change your preferred GP within your registered general practice at your convenience.

If you possess a valid Medicare or Department of Veterans’ Affairs (DVA) card and have visited us in person twice within the last 24 months, you’re now eligible for MyMedicare registration.

Opting not to register in MyMedicare doesn’t compromise the quality of care you receive from your healthcare providers.

If you’re interested in registering for MyMedicare, reach out to us or your GP. Further details are available at health.gov.au/mymedicare, and you can also access additional information by downloading our Fact Sheet.

MyMedicare serves as an optional patient registration system designed to formalize the connection between patients, their general practice, general practitioner (GP), and primary care teams.

Establishing a regular relationship with your GP through MyMedicare can contribute to improved health outcomes. When you register as a patient at our general practice and designate a preferred GP, the government provides additional funding to support your primary care team in delivering the necessary care.

Enrolling in MyMedicare is both voluntary and free, and it is open to Australians with a Medicare card or Department of Veterans’ Affairs (DVA) card.

It’s important to note that MyMedicare is solely a registration system and does not store any of your clinical health information. Your clinical health data will continue to be maintained in your My Health Record, if you have one.

Rest assured, all personal information recorded in MyMedicare, including your chosen healthcare providers, is securely stored, and your privacy is diligently maintained. For more details on privacy, you can review the MyMedicare Privacy Notice at health.gov.au/mymedicare-privacy.

You are eligible to enroll in MyMedicare through our practice if you meet the following criteria:

1.Possess a Medicare card or Department of Veterans’ Affairs (DVA) card.

2.Have had two face-to-face visits recorded at our practice within the last 24 months.

Individuals facing hardship, such as those experiencing domestic and family violence or homelessness, will be exempt from all eligibility requirements.

Parents or guardians, along with their children, can be registered at the same practice if one of them meets the eligibility criteria and is already registered. For children under 14 years, a parent or guardian must complete the registration process and provide consent on their behalf.

Young people aged 14-17 years can independently register and provide consent without the involvement of a parent or guardian.

Era Health Doctor Melbourne CBD is enrolled in MyMedicare.

Starting from October 1, 2023, you have the option to register for MyMedicare. To designate us as your preferred MyMedicare practice, you can follow these steps:

Online Registration: Register through your Medicare Online Account or the Express Plus Medicare Mobile app. Subsequently, our Practice staff will acknowledge your registration in the MyMedicare system.

or

Assisted Registration: You may initiate the registration process in MyMedicare through our practice, or you can instruct us to do so. This will initiate a registration prompt in your Medicare Online Account or Express Plus Medicare Mobile app, where you can finalize the registration.

or

Paper Registration:

Complete the MyMedicare Registration Form at our practice. By signing the form, you provide consent to participate in MyMedicare. Our practice staff will then finalize the registration in the MyMedicare system.

A successful registration in MyMedicare requires consent from both you and your practice, formalizing your relationship through one of the aforementioned processes.

Upon completion of your MyMedicare registration, your chosen practice and GP information will be visible on your My Health Record. This ensures that all healthcare professionals involved in your care, such as those at public hospitals, are informed about your regular care. If you prefer not to display your MyMedicare registration on your My Health Record, you have the option to select this preference in your My Health Record settings.

It’s important to note that MyMedicare registration does not restrict your access to care from other GP practices and healthcare providers.

Enrolling in MyMedicare is both voluntary and free.

By registering on MyMedicare, you inform your primary healthcare provider that they are your chosen regular care team. This choice brings additional government funding to support them in delivering the necessary care you require.

As a MyMedicare participant, you gain:

1.Improved continuity of care with your registered practice or GP, leading to enhanced health outcomes.

2.Extended Medicare Benefit Scheme (MBS) funding for telehealth consultations with your GP.

3.Incentives for prolonged MBS telehealth consultations, especially for children under 16, pensioners, and concession card holders (effective from November 1, 2023).

4.More frequent visits from your GP and improved care planning for residents in aged care homes (commencing August 2024).

5.Streamlined access to more suitable care within general practice for individuals who frequently visit hospitals (anticipated from mid-2024).

The extended funding for telehealth consultations can be particularly advantageous for individuals seeking support with mental health, those with disabilities, older Australians, and individuals residing in rural or remote areas.

The Australian Government is committed to continually enhancing Medicare, with ongoing benefits for registered MyMedicare patients.

Even if you choose not to enroll in MyMedicare, you will still have access to the same high-quality care from your healthcare providers.

Initiate a conversation with your regular general practice or GP to explore the benefits of registering in MyMedicare, or gather more information at health.gov.au/mymedicare.

If you possess both a DVA Veteran Card and a Medicare card, you have the option to register using either card. It’s important to note that you can only have one registration, and it will be applicable to any relevant Medicare and/or DVA-funded service, irrespective of the card you use for registration. If you opt to register in MyMedicare using a DVA Veteran Card, you will be required to fill out a registration form at your selected practice.

MyMedicare doesn’t bind you to a specific healthcare provider.

You have the flexibility to switch your preferred general practitioner (GP) within your registered practice at any time.

Opting out of MyMedicare is an option available to you whenever you choose.

If you meet eligibility criteria and the new practice is registered, you can also enroll with a different practice.

Registering with a new practice automatically cancels any prior registration and notifies the relevant practice.

To withdraw your MyMedicare registration, you can:

1.Use your Medicare Online Account or Express Plus Medicare Mobile app to initiate withdrawal.

2.Inform your GP or practice of your decision to withdraw your registration from MyMedicare.

3.Contact Services Australia directly and request the withdrawal of your MyMedicare registration.

Upon enrollment in MyMedicare, rest assured that your personal information will be safeguarded, ensuring the maintenance of your privacy.
